Remarks by Johan Visschedijk: "Consolidated used the first PB2Y-2 as a test bed, first flown on November 22, 1940, the aircraft was later delivered to Patrol Squadron 13 (VP-13) at NAS North Island, San Diego, California and coded "13-P-1". Subsequently it was used to provide shuttle service between NAS San Diego and NAS Pearl Harbor, Hawaii for high priority cargo and VIPs.

On 6 May 1942, shortly after take off from NAS Pearl Harbor, the No. 3 engine lost power, the pilot, Lieutenant Bowen F. McLeod, initiated an emergency landing. In the process of landing the left wing tip float hit a channel marker, on landing the aircraft made one surge and struck the corner of the dock. This sheared off the right float and tore two holes in the hull, the aircraft sank in about 20 ft (6 m) of water. The ten crew and eleven of the thirteen military passengers sustained injuries from minor to serious, while two passengers were killed. The aircraft was written off."
