05/15/2007. This was the third of three Twin Otter prototypes and registered as CF-SUL-X it was first flown in 1965. In 1967 it was sold to Northern Consolidated Airlines (NCA) and registered as N4901 it served with NCA till it was registered to Wien Alaska Airlines in March, 1968, after the merge of the two airlines.

It crashed at Minchumina, Alaska on March 13, 1969, after the pilot experienced spatial disorientation in a whiteout. One of the four passengers was killed. It was repaired and served with Wien Alaska for several years. It was seen parked engineless at Anchorage in June 1979. Thereafter it has been operated in Canada, registered as C-GKAZ to Carter Air Services at Hay River, Northwest Territories.
